Book an Appointment
Brain, spine and vascular consultations in Flint, Michigan with Dr. Shah-Naz Khan. You do not need a referral to contact us.
Three ways to get in touch
Call the office
(810) 212-4100
Monday to Friday, 8:00am to 6:00pm.
Best if you want to talk through symptoms, timing, or whether your insurance needs a referral.
Send an enquiry
Use the enquiry form and choose Appointment Request or Second Opinion as the enquiry type.
Available at any time. Please keep clinical detail brief and do not attach records.
Referring physicians
Clinicians referring a patient should use the dedicated referral process, which includes a downloadable form and fax details.
Not every patient needs one. Whether a referral is required depends on your insurance plan rather than on this practice, and if one is required in your case we will let you know and guide you through what to do next.
What happens after you get in touch
We review your enquiry
Your symptoms, any imaging that already exists, and the question you want answered. Enquiries are prioritised by clinical urgency.
We check the practical details
Whether your plan needs a referral or prior authorisation, and which records and images we need before the visit.
We schedule your consultation
And tell you exactly what to bring, so your appointment is spent on your diagnosis rather than on paperwork.
You see Dr. Khan
History, a focused neurological examination, a review of your imaging, and a clear explanation of your options and their timing.
What to bring
A little preparation means the consultation can focus on what matters: your diagnosis, your options, and your next step.
Paperwork
- Completed medical history form
- Photo identification and insurance card
- Medication list, including dosages
- Reports from recent visits or tests
If you have had imaging
- MRI, CT or X-ray images on CD or USB
- The written radiology report for each scan
- The date of each scan and where it was done
- Prior operative or hospital discharge summaries

What to expect at the visit
A proper history
What your symptoms are, how they began, how they have changed, and how they affect your daily life.
A focused examination
A neurological examination directed at the problem, not a rushed once-over.
Your imaging, explained
What the scan actually shows, what is uncertain, and what it means for you in plain language.
A clear plan
Your options, surgical and non-surgical, with the timing and the reasoning behind the recommendation.
Dr. Khan recommends an operation only when it is genuinely in the patient's interest. Many appointments end with monitoring, non-surgical treatment, or further investigation instead, and that is a legitimate outcome rather than a wasted visit.
If your symptoms are severe right now
Do not wait for an appointment. Call 911 or go to an emergency department if you have sudden weakness, facial droop, loss of consciousness, a seizure, sudden loss of vision, a sudden and severe headache, or loss of bladder or bowel control with back pain.
Common questions
Do I need a referral to book an appointment?
Not usually. Most patients can request an appointment directly. Whether a referral is required depends on your insurance plan. If one is required in your case, we will let you know and guide you through what to do next.
What are your clinic hours?
The clinic is open Monday to Friday, 8:00am to 6:00pm. You can call (810) 212-4100 during those hours, or submit an enquiry through the website at any time.
What should I bring to my first appointment?
A completed medical history form, photo identification and your insurance card, a current medication list including dosages, any imaging on CD or USB with the radiology report for each scan, and any prior operative notes or hospital discharge summaries.
Does a consultation mean I will need surgery?
No. A consultation is an assessment, not a commitment. Dr. Khan recommends an operation only when it is genuinely the right step, and many appointments end with monitoring, non-surgical treatment, or further investigation instead.
Can you review a scan taken at another hospital?
Yes. Bring the images on CD or USB along with the radiology report. Reviewing outside imaging is a routine part of both first consultations and second opinions.
Can I send my medical records through the website?
Please do not. The forms on this website are not a secure channel for health information. Bring documents to your appointment, arrange delivery through your current practice, or call the office and we will tell you the safest way to send them.
