What to include with your referral
Complete referrals support accurate triage and reduce delays. Please provide reports and details on where imaging can be accessed. Do not submit patient records via website forms.
Clinical details
- Primary complaint, onset, progression, and functional impact
- Key neurological findings and red flags
- Relevant comorbidities, anticoagulation, and medication list
- Specific question for consultation, opinion, management, or procedure
Imaging and investigations
- Radiology reports for MRI, CT, CTA, MRA, angiography, or spine imaging
- Date performed and imaging facility details
- Relevant labs or cardiac workup when stroke or vascular risk is suspected
- Operative history and prior specialist notes if applicable
How referrals are triaged and scheduled
Referrals are reviewed for urgency and completeness. Where key information is missing, we may request additional details to support safe prioritization.
