What to include with your referral

Complete referrals support accurate triage and reduce delays. Please provide reports and details on where imaging can be accessed. Do not submit patient records via website forms.

Clinical details

  • Primary complaint, onset, progression, and functional impact
  • Key neurological findings and red flags
  • Relevant comorbidities, anticoagulation, and medication list
  • Specific question for consultation, opinion, management, or procedure

Imaging and investigations

  • Radiology reports for MRI, CT, CTA, MRA, angiography, or spine imaging
  • Date performed and imaging facility details
  • Relevant labs or cardiac workup when stroke or vascular risk is suspected
  • Operative history and prior specialist notes if applicable

What is the best way to ensure timely triage?
Include the clinical question, neurological findings, and relevant imaging reports. If there are emergency red flags, direct the patient to emergency care.
Do you require imaging on disk?
Reports support triage. For definitive planning, access to images is often helpful. Provide the imaging facility and dates so images can be obtained when needed.
Will you communicate recommendations back to the referring physician?
Yes. We provide clear documentation of assessment, rationale, and defined next steps to support co-management and continuity.
Can referrals be for opinion only?
Yes. Many referrals are for confirmation of diagnosis, appropriateness of surgery, timing, or an alternative pathway such as monitoring or further workup.
