To help us prepare for your visit, please download and complete the appropriate medical history form before your appointment. Bringing completed paperwork allows your consultation to focus on your symptoms, imaging, and treatment planning.

Spine Patient Medical History
For patients being evaluated for spinal conditions including back pain, disc herniation, spinal stenosis, instability, or related neurological symptoms.
Cranial / Brain Patient Medical History
For patients being evaluated for brain tumours, vascular conditions, headaches, seizures, trauma, or other cranial neurological concerns.
If you are unable to print the forms, arrive 20 minutes early so they can be completed in the office.
A Simple Checklist Before Your Appointment
A little preparation helps us use your time well. If you bring the items below, we can focus the consultation on what matters most, your diagnosis, your options, and your next step.
Bring With You
- Completed medical history form
- Photo identification and insurance card
- Medication list (including dosages)
- Relevant reports from recent visits or tests
If You Have Imaging
- MRI, CT, or X-ray images on CD or USB
- Radiology report for each scan
- Scan dates and imaging facility details
- Any prior operative or hospital discharge summaries
What to Expect
- Review of history, symptoms, and imaging
- Focused neurological examination
- Clear explanation of options and timing
- A plan for next steps or follow-up
If you are experiencing severe symptoms such as sudden weakness, facial droop, loss of consciousness, or a severe sudden headache, seek emergency care immediately.
